Eden Forms

Chris has forwarded me the following documents that you need to complete in order to attend the trip:

Personal Details Form

Diversity Monitoring Form

They must be filled in and signed ASAP

    on the personal details form, there's quite a lot of information we've not been told yet - the second page of it, for example, concerning cars and teams, etc. shall I just try and ask chris about this on thursday?

    that page isnt really relevant to students....that's just for the driver
